• Application for the State Exam/Thesis defence - Biochemistry, Biophysics, other programmes
• Student´s international mobility form - filled form has to be sent to the Vice-Dean for doctoral studies (This email address is being protected from spambots. You need JavaScript enabled to view it.) to register the mobility before leaving for the stay. Students going through Erasmus+ programme do not need to submit this form, their internship is administered and registered by Mgr. Barbara Okosy.
• Form PhD Annual report will be send to students by the Department for Student Affairs, guidelines (for advanced students), guidelines (first time form-filling)
• Request for the increase of regular scholarship in the doctoral study
• Request for reimbursement of teaching* in the winter/summer semester of the academic year (docx, pdf)
• Assignment of bachelor/master thesis (docx, pdf)
• Request for consent to the use of classified information in a bachelor's/master’s thesis 
• Confirmation of an internship form (docx)
• Request for postponed publication of a thesis (docx)
